Accepted Insurance Providers

It is our practice policy to bill all insurance companies as a courtesy to our patients. Our experienced billing team will file your claim for you and bill you for the remaining balance on your account.

We currently accept most insurance plans. Please call the office to check for yours.

 

Benefits and Eligibility

To determine your insurance benefits and eligibility please contact your insurance carrier directly. Note that you are ultimately responsible for payment for all services that are not covered by your insurance plan.

HMO Procedures

We are contracted with most HMO Insurance Companies. An HMO plan may require that you obtain a referral from your primary care physician’s office. If this is the case, please contact your assigned primary care doctor, whose name will be listed on your insurance card.

Payment Options

Payments can be made at our office or sent to our Business Services office in the form of checks, or major credit card.

We will bill your insurance as a courtesy to you. Services that your insurance does not cover or reimburse will be your responsibility.
